Service Description

Case Management services are for Children and Pregnant Women (CPW) who receive Medicaid benefits. The ages for children are birth through 20 years of age and pregnant women of any age who have a health condition, health risk, or high-risk condition. The patient must be eligible for Medicaid and be either: A child age birth through 20 years with a health condition or at risk for a medical condition, illness, injury or disability that limits the child’s ability to function or engage in activities or social roles compared to healthy same-age peers, or A pregnant woman of any age with a medical or psychosocial condition that increases the risk for pregnancy or postpartum complications. Many of these patients have complex needs that range from specialty referrals and medical equipment/supplies to educational, psychosocial, financial, and transportation services. Services are furnished to assist members in gaining access to needed medical, social, educational, and other services. Case managers; are licensed social workers or registered nurses, community health workers, and doulas who help patients and their families connect with these outlined services. For children, we offer a variety of social skills training, while advocating for educational needs and community support. Doulas don't perform any medical tasks. Our main priority is to encourage the mothers to advocate their needs during the course of their pregnancy while acting as extra support during labor and assisting sometimes in the early months after childbirth. We provide laboring techniques for women to remain comfortable and calm using various methods, including suggesting comfort measures and optimal positions for labor and pushing. Postpartum doulas provide families with information and support on infant feeding, emotional and physical recovery from childbirth, infant soothing, and coping skills for new parents. They might also help with light housework, fix a meal, and help incorporate an older child into this new experience. An end-of-life doula is someone who provides companionship, comfort, and guidance to those planning for death, diagnosed with a terminal illness, or facing imminent death. Our doulas offer nonmedical holistic support encompassing emotional, spiritual, and practical care while suggesting resources to help children and pregnant women improve their circle of care to make informed decisions in a supportive nurturing environment.
